Requirements to Expedite New or First Time U.S. Passports

You will be considered a first time or new passport applicant if:

If you are applying for a first time U.S. passport.
Your previous passport is expired and it was issued over 15 years ago.
Your passport was issued when you under 16 years.

Complete DS-11 passport application form, fill it online. But do not sign...
Two passport photos. 2" x 2" white background. (Photo service is available at our office.)
Original/certified US birth certificate or an expired US passport or Original naturalization certificate.
Proof of Identity (A Valid Driving License, Government Issue Photo I.D).
